Healthy Lifestyles programs at the Y go beyond physical activity to focus on activities that enhance overall health and well-being. We don’t want to just help you make a temporary change. We want to help you on the journey of self discovery and learning strategies for successful and sustained behavior change in a positive, supportive and accepting small group environment. To provide the highest level of quality, most of our Healthy Lifestyles programs have been developed in collaboration with a variety of national and local health care, academic and community partners.

Below are two FREE online self-assessments to help you gain insight into your current health and well-being. The results of your assessment can provide a great road map for developing a plan for lifestyle change and a healthier future.  Your assessment might also help direct you towards the best Y program to support you along the way. At the Y we have healthy lifestyles programming that can help you chart your progress towards your goals for health and well-being.

 

FREE Health and Lifestyle Assessment            Ages 18+
The Stanford Health and Lifestyle Assessment is an online survey designed to help you assess your current health and future health risks, as well as assess your readiness to make lifestyle changes. This assessment is a perfect starting point for anyone contemplating lifestyle changes and looking for a personal, confidential and quality assessment with an emphasis on health and disease prevention. After completing this free online assessment, you’ll be able to print out a report that outlines your personal health risks and make suggestions related to nutrition, stress, fitness and finding support for staying healthy. After you complete the assesment, please contact your local center to set up an appointment with a wellness expert to discuss your results in further detail and map out a plan or check the program guide for a healthy lifestyle class that fits your needs. 


FREE Dimensons of Well-Being Self Assessment            Adults
The Living Compass Self questionnaire is an online self-assessment designed for any person who wants to take a look at their total well-being, spirit, mind and body. This assessment helps you explore 8 different dimensions of wellness. These are 8 common areas of your life that can either become causes of stress and dissatisfaction, or become sources of happiness and life satisfaction. After completing your assessment, your responses are plotted on a wellness wheel – called the Living Compass – and when you print it out, it provides a visual picture. It is a snapshot of your wellbeing at this very moment. It might be different if you had taken it a month ago, or were to return to take it a month from now as life is always changing. Taking an assessment like this a few times a year is a good practice to get into as it helps you make sure you are heading in the right direction. If you are inspired to really begin a focused effort in enhancing wellbeing and life balance, the Living Compass group program or Living Compass one-on–one coaching sessions might be perfect for you.
